Method #1. The IPC will be sending out an e-mail to all individuals who have completed the IPC’s online Poison Prevention Education Course before the Sweepstakes Period ends and who had provided the IPC with a valid e-mail address. This e-mail will invite participants to complete an online survey. To enter, participants must follow the instructions in the e-mail to access the survey, answer all survey questions, and provide all required information. The answers to the survey will have no effect on a participant’s ability to enter or win. However, incomplete surveys are not eligible. Also, a survey that has been tampered with or altered, or completed by a script, macro or use of automated device will be deemed incomplete and will not be eligible.  Upon completion of the survey, the participant will receive one (1) Sweepstakes entry (“Survey Entry”). Limit one (1) Survey Entry per Poison Prevention Education Resource Center (“PPERC”) ID. Participants submitting a Survey Entry are subject to all notices posted online including but not limited to the Sponsor’s Privacy Policy (see “Privacy” below).

Method #2. To enter the Sweepstakes without completing the survey, hand print your name, complete address, city, state, zip code, daytime phone number, birth date, e-mail, and PPERC ID on a single, letter size sheet of paper (i.e., 8.5 by 11 inches) and place inside an envelope and mail it to: Illinois Poison Center ATTN Sweepstakes, 833 W. Jackson Blvd., Suite 610, Chicago, Illinois 60607 (“Alternate Method of Entry” or “AMOE Entry”). A participant’s AMOE Entry must be postmarked on a date that is within the Sweepstakes Period to be eligible. Limit one (1) AMOE Entry per participant. AMOE Entries that are mutilated, illegible or incomplete are not eligible. All entries received become the sole property of Sponsor and will not be returned.  Sponsor will not be responsible for illegible, late, lost, mutilated, misdirected, postage due or stolen manual entries. Information submitted through an AMOE Entry is subject to Sponsor’s Privacy Policy (see “Privacy” below).

LIMIT:  Limit one (1) Survey Entry per Poison Prevention Education Resource Center (“PPERC”) ID or limit one (1) AMOE Entry per participant.  Participants submitting both a Survey Entry and AMOE Entry will be limited to one entry.

Sponsor will contact a potential Grand Prize winner via email and/or phone and the potential Grand Prize winner will be required to provide their mailing address within fourteen (14) calendar days via email to a Sponsor-provided email address. If a potential winner is deemed a minor in his or her state of primary residence, the minor’s parent(s)/legal guardian must provide their name, mailing address, and the minor potential winner’s email within fourteen (14) calendar days via email to the Sponsor-provided email address and the Grand Prize will be awarded in their name. The Sponsor will send the Grand Prize to the mailing address provided by the winner and/or the winner’s parent(s)/legal guardian within thirty (30) days of the potential winner (or his/her parent(s)/legal guardian) providing their mailing address (and, if applicable, the minor potential winner’s email), subject to Sponsor’s determination of the winner’s compliance with these Official Rules.
